These forms are required by the State of Connecticut. The Health Exam Form requires a physician’s signature. Exams are valid for 3 years. In addition, the Administration of Medication forms are required for children with asthma, or with food and/or drug allergies. Please download, print and complete these forms and bring them with you to Camp Check-In.
